What do we do?

 

Changing Trax is a short-term Crisis Intervention Service funded by Newcastle Neighbourhood Renewal. The aim is to create a positive change by working intensively with families where substance and/or alcohol misuse is an issue.

Research indicates that crises last for about four weeks, and can act as a vehicle for change. Our intervention is focused on enabling people to learn and practise new skills and patterns of behaviour during this active period. We aim to achieve clear and measurable change so that children can remain safely in the home.

 

 

How do I refer?

 

Referrals need to come through Children’s Social Care by the child/children’s social worker. Therefore your first contact could possibly be with the children’s social worker to discuss the service we offer and whether it is appropriate. However if you feel you would like to discuss a case first or you think we can help you directly please contact us on the available number.

 

 

Who is the service for?

 

The service is for families where there is substance and/or alcohol misuse. Children’s Social Care will also be considering what action they need to take to safeguard the child/children in the family. Therefore children will need to be at active risk of accommodation or having their names placed on the child protection register. Children must also have an allocated social worker who will hold case responsibility during our intervention. Families must be in crisis and know that a referral is being made.

 

 

What will happen at referral?

 

A referral will be taken by a Changing Trax worker from the child/children’s social worker either over the phone or we will meet in person to take the referral. Once it has been established that it is an appropriate referral and family have agreed to our involvement we will make contact with the family within 24 hours and arrange to visit.

 

During the first 72 hours after referral, we will make an assessment of whether a Changing Trax intervention is appropriate. If we are unable to work with the family because we feel the dangers are too great, the intervention will end and the social worker will be informed. A report will also be written about why our involvement could not progress.

 

 

What services do we offer?

 

Workers will be available for families 24 hours a day, 7 days a week for a maximum of 4-6 weeks. We work with families at home and in the community providing a high frequency of contact with the family during this period.

 

There will be follow up visits at one, three, six and twelve months and we can offer booster sessions of up to 2 days to families.

 

There are three Changing Trax workers who each work intensively with two families at a time. As we provide an intensive service working with families at a critical time, we do not have a waiting list. If all three workers are at capacity, then the referral will not be accepted.
